When an employee of the Sioux City Community School District sustains an injury, it not only impacts that employee’s life, the injury impacts his or her family, and the continuity of the students we educate.  

What you should do if you are injured at work? 

Workers’ Compensation Injury Procedure 

  1. In the event of a severe injury, call 911. Otherwise, notify your supervisor or principal of your injury/incident. 

  1. For all injuries and near misses, call Company Nurse at 1 (888) 375-0284. Give them your employer name of Sioux City Community School District, with search code QT837. Please keep a record of your incident/claim number.

  2. Seek medical attention as directed by Company Nurse. You may be referred to self-care, an occupational health practitioner, urgent care, or to go to an emergency room.

  3. After your medical visit, you will be required to complete the necessary paperwork. Human Resources (HR) will be notified of the incident by Company Nurse and will provide you with the required paperwork. Please submit the paperwork within 24 hours to your principal, supervisor, or to HR. For assistance, or any concerns, call human resources at (712) 279-6692.   4. Attend any follow-up appointments as directed by the workers’ comp physician or nurse’s care plan until the injury/illness is resolved.

  5. Submit all duty restrictions and return to work documents to your principal, supervisor, or the HR department before your next work shift. For light duty restrictions, complete the temporary modified/light duty form with your principal or supervisor.

Note: If you choose to be treated by a personal doctor, you may not qualify for workers’ compensation insurance benefits and you may be responsible for all medical costs related to this incident. This is in accordance with Iowa Workers’ Compensation statute, Iowa Code Chapter 85.27.
